The Joppa Health Share Memberships means this website and any other services provided by Joppa Health Share, as may be selected by you or automatically delivered to you. “You” and “your” means the person or entity using the Joppa Health Share Membership and providing data in connection with the Joppa Health Share Membership, as an “Account Holder”. “Joppa Health Share” “we”, “us” and “our” means Joppa Health Share, its subsidiaries and affiliates.

Family and Friends Involved in Your Care: With your approval, we may from time to time disclose your information to designated family, friends, and others who are involved in your care or in funding for your care in order to facilitate that person's involvement in caring for you or funding for your If you are unavailable, incapacitated, or facing an emergency medical situation and we determine that a limited disclosure may be in your best interest, we may share limited personal health information with such individuals without your approval. Member Enrollment Request: An enrollee must first complete the Member Enrollment Application. During the enrollment process a Member Enrollment Application is required to give certain information (such as name, email address, demographic information, medical information, etc.). This information is used to review your enrollment, allowing Joppa Health Share the opportunity to ensure you and all enrollees meet the enrollment criteria per the Member Guidelines on our website.
Sharing of Information: We share aggregated demographic information with our subcontractors, where we have partnered with a third party to provide specific We will share names, or other contact information that is necessary for the third party to provide these services.
